Consent Form

When We were given the focus group task, We were also asked to produce a consent form. A consent form is a small list which you give to people on who you will be doing a questionnaire on. For example it had to include questions like 'do you understand the information that has been given to you?' This used because it makes it legal to allow you to present your name publicly without breaking the law of revealing someone's identity.

Here is an example of our consent form, Each question is used to certain example so that all areas are covered in terms on a legal case. At the bottom is two signatures, one is the person who this form was given too and that we have verification to use their names overall. The second signature is my signature that is used to show that I have seen their answers and that I understand that I cannot use their information without their verification.
